Legal in all 50 states

Five Wishes was written with the help of the American Bar Association’s Commission on Law & Aging. It meets legal requirements and is used in all 50 states. Federal law requires medical providers to honor patient wishes as expressed. Additionally, Five Wishes meets state-specific advance directive requirements. As noted on page 3 of Five Wishes, only four states (KS, NH, OH, NH) have additional requirements that may require the use of a state form.
