Five Wishes partners with Hospice Foundation of America, a national leader in online education for healthcare professionals, to offer these courses through their highly accessible learning platform. Courses can be completed at your convenience, at your pace, and from any location. Once you are registered, all materials and instructions are immediately available.

Five Wishes Presenter Training

  • Online learning module includes 1.5 hours of continuing education (nursing, social work, mental health counseling)

Audience: This course is appropriate for professionals and volunteers in any field or profession who want to deliver community education presentations about advance care planning using Five Wishes, including community outreach and education staff or professionals, attorneys, financial planners, faith community leaders or members, civic group leaders, speaker's bureau members, social workers, community health workers, and others.

This training is designed to enhance your knowledge, ability, and confidence in providing Five Wishes advance care planning educational presentations to communities. This course provides an introduction and overview of the definition, benefits, and characteristics of good advance care planning, and explains the importance of community education. In addition, the course describes the Five Wishes approach, and outlines the most important information to cover in delivering a presentation about Five Wishesin your community.

Five Wishes Facilitator Training

  • 3 online training modules include 3 hours of continuing education credits (nursing, social work, mental health counseling). It is the most advanced and comprehensive training option for clinicians.
  • Courses are delivered through Hospice Foundation of America's online learning platform and are available for one year from the day of purchase. Enrollment details are sent by email after purchase.

Audience:

This course is appropriate for clinicians who facilitate advance care planning conversations with individuals. Unlike the Presenter Training, which focuses on providing public presentations and education to groups of people, the Facilitator Training focuses on assisting individuals as they complete their advance directive. It is most useful for healthcare professionals who engage directly with patients, families, and caregivers.

Online training modules include:

1. Facilitator Foundations - 1 Continuing Education hour. This is the foundational Five Wishes Advance Care Planning course designed to enhance your knowledge and skills in facilitating compassionate, effective conversations with individuals, families, and decision-makers in healthcare settings. This course provides an introduction and overview of the benefits, components, and characteristics of effective advance care planning, and outlines how to use the Five Wishes framework to guide conversations and ensure they address what is most important in facilitating high-quality advance care planning. This course prepares learners for more advanced Five Wishes advance care planning courses.

2. Facilitator Communication Skills - 1 Continuing Education hour. Building on the course titled Five Wishes Advance Care Planning Facilitator Foundations, this Five Wishes Advance Care Planning course is designed to enhance your knowledge, ability, and skill in facilitating compassionate, effective conversations with patients, families, and decision-makers. This course provides an overview of essential communication and conversation skills, important areas for assessment and exploration, and special considerations for sensitively engaging in advance care planning discussions. You will learn behaviors and practices that demonstrate compassionate, person-centered communication, and help you effectively facilitate values-based end-of-life care discussions.
3. Planning for Serious Illness - 1 Continuing Education hour. This Five Wishes Advance Care Planning for Serious Illness course provides an overview of key concepts and skills for facilitating effective and sensitive advance care planning and goals of care conversations with people at various stages of serious illness. It outlines considerations to ensure that advance care planning and goals of care conversations empower and equip people to make medically-informed treatment and end-of-life treatment choices that are consistent with their goals and values, and are likely to be honored. This course is appropriate for clinicians who have completed the Five Wishes Advance Care Planning Facilitator Foundations course and the Five Wishes Advance Care Planning Facilitator Communication Skills course, or for those who have a firm understanding of the purpose, process, and components of effective advance care planning, and who possess a basic level of the communication skills involved.
