That's why Elevance has partnered with Five Wishes, the most popular advance directive in America.  Five Wishes is helpful for all adults – everyone over age 18 – so we hope you use it and share it with the people who are closest to you.  The process is simple. 

  • Choose a format

    English PDF, Spanish PDF, or Booklet Mailed.  If you choose a PDF, it will be personalized with your name and birthdate.  The personalized PDF can be printed and saved on your device.  If you prefer to have a hardcopy booklet, available in 33 languages, choose Booklet Mailed.
