Five Wishes in My State

If you live in the District of Columbia or most states, you can use the Five Wishes advance directive and have the peace of mind to know that it meets your state’s legal requirements. If you live in one of four remaining states (New Hampshire, Kansas, Ohio, or Texas) you can still use the Five Wishes advance directive but may need to take an extra step.

In Wisconsin, you need to attach a notice statement (we include a copy when you order Five Wishes). In Michigan and North Dakota, your agent will need to sign an acceptance form before they can begin to act. These aren't included in the Five Wishes document because your agent isn't required to complete the acceptance form at the same time that you complete your Five Wishes (but we do include a copy for you when you order Five Wishes).
